Sounds like poor contacts, dirty contacts, high impedance, dry joint ( poor solder connection ) etc. I know on ibooks there is impedance detection on the minijack output, this could be getting confused by dirt/oil/residue which would affect the output driver. Try a contact cleaner, non greasy.
Otherwise it's software based and a reset might do it.
If the minijack has got an bad solder connection then it'll need opening up etc big bucks!
