Polyfractus just released a new plug-in: PitchToMidi.

Hello there.
Sorry for non response. Unfortunately i didn't sell PI2M licence to Apple  I dont understand why it doesn't work with your cubase, i have checked the plug with Cubase 5 (demo) so many times and it works (other people are using PI2M without problem) Here we go again. Insert the PI2M plug on the audio track to convert. Set up option (FFT size, window size...) Select MIDI track to play or record. Start playing... Thats all.. Hope this help.
